This is a guide to Ultherapy PRIME compiled by Dr. Choi Byeong-geun of SHINE U Clinic Konkuk (3 minutes walk from Konkuk Univ. Station Exit 1).
Skin aging does not occur due to a single cause. Changes that naturally progress over time overlap with changes added by sun exposure. In this article, we will summarize how natural aging differs from photoaging, how UV rays weaken collagen, and where lifting intervenes.
Differences between natural aging and photoaging
Skin aging is largely explained in two ways.
- Natural aging: This is a decline in function over time, typically characterized by wrinkles, loss of elasticity, dryness, and decreased sebum production.
- Photoaging: These are changes added by UV exposure, with sagging, pigmentary changes, and a rougher surface observed on the face, neck, and back of the hands, which are exposed to sunlight.
It is reported that even at the same age, skin condition can vary greatly depending on the history of UV exposure.
How UV rays break down collagen
Prolonged exposure to UV rays increases an enzyme called MMP in the skin. MMP is an enzyme that breaks down proteins between cells, and it acts on collagen in the dermis, leading to a loss of elasticity and the formation of wrinkles.
UV rays are also known to inhibit enzymes that produce fat in subcutaneous fat cells, thereby reducing the amount of subcutaneous fat. As a result, further loss of elasticity and deepening of wrinkles may follow.

Why is it difficult to restore lost elasticity?
Once collagen is weakened, natural recovery is slow, so active management is often combined with prevention through blocking. Lifting devices are used to help restore elasticity by stimulating the SMAS layer and inducing tissue regeneration.
Why we distinguish between deep and superficial layers
Aging does not end with a single problem; sagging, wrinkles, and loss of elasticity appear together. Therefore, an approach that combines devices with different action layers is considered.
- Ultrasound-based devices: Deliver thermal energy to the SMAS layer of the skin to address deep sagging.
- Radiofrequency-based devices: Delicately address superficial elasticity in contoured areas such as around the eyes, mouth, and jawline.
Since skin thickness, wrinkle severity, and fat distribution vary from person to person, it is recommended to determine the combination based on a diagnosis.
Conclusion
While natural aging cannot be prevented, photoaging is approached by slowing its progression through blocking and management. The appropriate lifting combination varies depending on the skin condition, so a diagnosis comes first.
