This is a comparative guide to non-surgical lifting procedures, compiled by Dr. Choi Byeong-geun of SHINE U Clinic Konkuk (a 3-minute walk from Konkuk Univ. Station, Exit 1).
One of the most frequent questions we receive in the consultation room is, "Should I get Ultherapy or Thermage?" Because both procedures are grouped under the term 'lifting,' it's easy to misunderstand them as similar. However, in the examination room, we see that the problems these two procedures solve are completely different. It's simplest to view Ultherapy as a tool to lift deeply sagging tissue, and Thermage as a tool to tighten loose skin texture and elasticity.
1. Operating Principle: Ultrasound vs. Radiofrequency
The biggest difference lies in the type of energy delivered to the skin.
Ultherapy PRIME uses HIFU, which is High-Intensity Focused Ultrasound. It concentrates heat in a dot-like pattern at depths of 1.5mm, 3.0mm, and 4.5mm in the dermis and SMAS layer, stimulating the sagging SMAS structure itself to lift and encouraging new collagen to form in that area. The core is to contract the same layer that surgical lifting pulls, but with heat and without incision.
Thermage FLX broadly delivers monopolar radiofrequency (RF) to the dermis and subcutaneous fat layer. Instead of focusing on a single point, it heats and contracts dermal collagen over an area, making it strong for elasticity recovery and texture improvement. 600 shots are the basic recommended amount per session, creating changes in the overall facial texture.
Although both are grouped under the term 'lifting,' understanding Ultherapy as 'lifting up' and Thermage as 'tightening' causes the least confusion when starting a consultation.
2. Key Difference: What Each Targets
| Item | Ultherapy PRIME | Thermage FLX |
|---|---|---|
| Energy Type | High-Intensity Focused Ultrasound (HIFU) | Monopolar Radiofrequency (RF) |
| Target Layer | SMAS layer · deep dermis (dot-by-dot) | Dermis · subcutaneous fat layer (area-by-area) |
| Strengths | Lifting deep sagging | Texture · elasticity recovery, contour tightening |
| Effect Onset | Immediate after procedure + gradual over 2-3 months | Immediate tightening + gradual over 3-6 months |
| Pain | Stimulating sensation in areas near nerves | Widespread burning sensation |
| Recommended Cycle | Once every 1 to 1.5 years | Once every 1 year |
| Recovery | Immediate return to daily life | Immediate return to daily life |
Since Ultherapy reaches the SMAS layer, its effects are long-lasting and deep. Thermage heats the entire dermis, resulting in a more refined texture and contour. This difference is why both procedures are often recommended together.
3. Which Patients Are Recommended Ultherapy?
The common characteristics of patients for whom Ultherapy is primarily recommended in the examination room are as follows:
- Those whose cheeks below the cheekbones have started to sag, creating shadows around the mouth.
- Those whose jawline has collapsed, blurring the boundary between the double chin and the side of the face.
- Those who clearly show sagging cheeks when viewed from the side.
- Those whose primary concern is sagging itself, with relatively maintained elasticity.
In these cases, Ultherapy, which can directly stimulate the SMAS layer, is a higher priority. A clear line must be established for the effects of other procedures to last longer.
4. Which Patients Are Recommended Thermage?
The characteristics for which Thermage is primarily recommended are slightly different:
- Those who frequently check the mirror due to texture, pores, or fine wrinkles rather than sagging.
- Those whose makeup looks uneven and whose skin feels rough.
- Those whose skin has suddenly sagged after childbirth or dieting.
- Those who are concerned about areas with reduced elasticity over an area, such as the forehead or around the eyes.
Thermage, which broadly heats the entire dermis, is more natural for creating changes at the texture level. If you wish to focus specifically on the eye area, Thermage FLX for Eyes (225 shots, dedicated to the eye area) is available as a separate option.
5. The Most Common Misconception in the Examination Room
We often receive the question, "If I get one of these procedures, I won't need the other one ever again, right?" These two procedures target different layers and are complementary, not interchangeable.
Patients who have repeatedly received only one type of procedure from elsewhere sometimes feel that changes in texture or sagging have plateaued. At this point, adding the other tool can restart the progress. The Korean Laser Dermatology and Hair Society's non-surgical lifting guide also recommends combining HIFU and RF as an optional treatment. In the examination room, we also find that distributing the two tools according to the patient's ratio of sagging and elasticity, rather than sticking to just one, leads to more stable results.
6. If Receiving Both, What Is the Best Order?
When performing both, the following sequence is generally followed:
- Ultherapy PRIME first — to establish the framework of the SMAS layer.
- 2-4 week interval to confirm skin recovery.
- Thermage FLX added — to finish the texture and elasticity over an area.
- 3-6 month interval to observe gradual changes in the dermis and subcutaneous fat.
If sagging is not deep or only texture is a concern, Thermage alone may be started. If sagging is the priority, Ultherapy alone may be started, and after observing changes in one aspect, a decision can be made whether to reinforce with the other. This flow is also common in the examination room.
7. Post-Procedure Care
Both Ultherapy and Thermage allow for immediate return to daily life, but adhering to the following points can help ensure stable results:
- On the evening of the procedure, wash your face gently and avoid rubbing.
- Skin tightness, redness, and mild itching for a few days are natural reactions and will gradually improve.
- Avoid makeup on the day of the procedure; you may proceed as usual from the next day.
- Use plenty of moisturizer and sunscreen.
- Avoid alcohol, smoking, saunas, and strenuous exercise for one week.
8. Frequently Asked Questions
Q. Can I receive treatment for my entire face at once? The entire face + submental area can be treated in one session. As the area widens, it is important to distribute the shots sufficiently for balanced results.
Q. When are the effects most visible? Some immediate changes are reported right after the procedure. However, the most distinct difference is reported in front of the mirror around 2-3 months, when the collagen regeneration cycle is active. It gradually becomes more refined up to 6 months.
Q. At what age is it best to receive the first treatment? Some people receive it preventatively before sagging begins, while others receive it for recovery after it has already progressed. In the examination room, most consultations are for individuals who start to show mild signs in their mid to late 30s.
Ultherapy PRIME and Thermage FLX are not tools where one is superior to the other; rather, they are two tools, each excelling at different tasks. If the most concerning change you see in the mirror is sagging, you can find detailed information about the procedure in Ultherapy PRIME Details. If it's texture and elasticity, you can check Thermage FLX Details. If you wish to focus on refining just the eye area, we recommend also looking at Thermage FLX for Eyes Information.
Consultations begin by discussing what changes you notice first in the mirror. You can inquire directly via Chat for non-face-to-face consultation.
